Note added on May 29: In response to queries that I’ve received, I’ve added a few margin notes to explain Bank jargon and common knowledge that I relied on when I was writing to insiders. I have also cleaned up at least some of the far too numerous typos. This is keeping with my claim in the post Writing that when there are more potential readers, an author should spend more time improving the exposition.
